No matter who gets injured at an AYSO event, we must document the incident to protect the injured person, you, and our Region. Medical and liability insurance is not only for the Player, but also for our Volunteers. Filling out an Incident Report immediately after an injury, property damage or any other serious incident will give you an opportunity to capture facts and peoples' contact information before witnesses leave the field. Incident Reports may be filed by any AYSO volunteer, but for injuries or incidents during training or a game it is best if the coach files the Report.
Incident Reports should be completed by any Region staff witnessing any serious incident involving, but not limited to:
- Injuries, especially during training or a game
- Threats of bodily harm
- Fighting whether or not a serious injury occurs
- Property damage
- Hospitalization of a participant
- Law enforcement summoned
- Service or notification of pending lawsuit

If the incident involves an injury send an email immediately to the Safety Directory, firstname.lastname@example.org explaining the situation and giving as much contact information as possible. The Safety Director will follow up with you and help manage the reporting process, and file the report with National.
