Team Uniform Control Sheet

Each player gets 2 jerseys, a white and a blue, 1 pair of shorts and 2 pairs of socks.  The jerseys and shorts must be returned at the end of the season.  

Parents are responsible to return uniform jerseys and shorts to the Team Manager within 7 days of the last scheduled game.    

Parents:  Your initials below signify that you acknowledge and agree to return the 2 jerseys (blue/white and white/blue) and the pair of shorts in good condition to your coach at the end of the season.  Also, that the jersey and shorts will only be worn at games, not during practices nor any other non-AYSO or AYSO function, than games or scrimmages.  Parents acknowledge that they will Pay for Damaged or Lost Uniform if the damage is not normal soccer wear and tear.
