Insurance Certificates


This page is used to generate and hold certificates that are required for the City of Palo Alto and PAUSD.  The insurance certificates are generated online by the RC.  They are then attached to this page.  The Non-Profit letter form the IRS is also attached.
Instructions to the RC to Issue Certificates
Each year we must supply PAUSD and the City of Palo Alto with certificates from our insurer covering them for issues related to AYSO use of their facilities.  Those certificates are issued on demand, using the AYSO website.


Select Certificates of Insurance from the left menu.

Once logged in, search for each of the two "City of Palo Alto" and "Palo Alto Unified School District", and for each, check the box and click "Issue" at the top  of the page.  A PDF of the certificate will be emailed to the recipient.
